Northeast Delta Dental in-network providers complete all of the paperwork for you -- and Delta Dental pays your dentist directly. To locate a Northeast Delta Dental in-network provider or obtain a claim form, please visit www.nedelta.com and select plan type Delta Dental Premier or Delta Dental PPO

ID Cards

Northeast Delta Dental will issue an initial set of plastic ID cards to new primary subscribers only at the time of enrollment. They do not issue replacement cards and any future name or plan changes will be reflected on the subscriber’s electronic ID card, which is available through their site at www.nedelta.com/Patients. The ID card includes the primary subscriber’s name and ID number (the ID number is the same for all covered family members).

Delta Dental Mobile subscribers have access to dentist search, claims and coverage, ID cards and more right on their mobile device.

Health through Oral Wellness (HOW®) patients who are at risk for certain conditions may be eligible for additional preventive benefits at not additional cost to them.

Explanation of Benefits (EOB) for subscribers who prefer to receive hard copies of EOBs via standard mail may elect to do so on this site or by calling Delta Dental's eligibility department at 603-223-1230.
