New Student Health Forms

• All Bowdoin students must comply with Maine’s Immunization Law as well as requirements put in place by the College. 
• Obtain and review your immunization record from your healthcare provider (HCP) ASAP.
• Make an appointment to receive missing immunizations over the summer.
• Submit your immunization record via the Student Health Portal as soon as possible.
• Delays in submitting your immunizations could lead to class registration being held.
• Medical exemptions are allowed; however, students who are not immunized will be asked to leave campus for an extended period in the event of a disease outbreak.

• Physical Exam Form
  
  • This form is strongly recommended for new students and required for all athletes. Students not participating in athletics may submit a physical performed within the last 18 months.
  • All athletes should refer to Athletic Training for athletic requirements.
• Immunization Form
  • Ask your HCP to ensure that your immunization record meets the requirements and complete your immunization form. If you require additional immunizations, you should plan to receive them prior to arrival on campus.
  • In some cases, students without evidence of immunization can submit blood test results that show immunity against varicella, measles, mumps, rubella, and hepatitis B. 
  • Ideally, immunizations are received prior to arrival at Bowdoin; however, Health Services will work with students who need help receiving vaccines that may be difficult to receive at home (i.e. international students)
• TB Form(s)
  • Students answering YES on the Tuberculosis Screening Questionnaire on the student health portal must have their HCP complete Tuberculosis Testing Form.

Log into the Student Health Portal.

Register for the student health portal using your username@bowdoin.edu address. Enter your password for your Bowdoin account.

• In the student health portal select the My Forms tab.
• Complete the Medical History and Wellness Form.
• Complete the Immunization History Form using the immunization documentation obtained from your HCP.
• Read and sign the Consent Form.
  • Must be signed by the student.
  • If you will be under 18 when you arrive on campus, your parent or guardian must sign.
• Complete the Tuberculosis Screening Questionnaire.
• Complete the HS Plan Information Form.
• The Physical Form PDF and Immunization Form PDF can also be printed from the My Forms page.

Documents with the following file formats can be uploaded: JPEG, TIF, PNG, PDF, BMP, and GIF. Each file should not exceed 6MB.

Please upload the following documents via the upload tab embedded in your Portal Forms. ​​

• Immunization Form
  • Completed and signed by HCP.
  • Immunization records from HCP office are acceptable if they fulfill requirements and are in English
  • Upload within the Immunization History Form after entering your dates of immunizations. ​
• ​Physical Exam Form
  • Completed and signed by HCP
  • Athletes must submit Bowdoin Physical Exam Form and physical must be performed after March 1st, 2023. This is an NCAA requirement.
  • Upload within the HS Physical Form and submit. ​
• ​TB Form(s) — only if indicated by screening
  • If indicated by your TB Screening Form, the TB Testing Form must be printed and completed by your HCP.
  • Upload within the TB Screening Form and submit. ​​
• Insurance Cards
  • If prompted by HS Plan Information Form, take photos of the front and back of card.
  • Upload images within HS Plan Information Form.
  • If insurance changes during the school year, please upload a new card.
