Mandatory Forms on Site
Students must fill out these three required OCS forms at the commencement and conclusion of their time away.

At the start of each semester away, we require students to complete the following two online forms.
These forms are short, mandatory, and online, not to mention critical to academic approval and support in case of emergency. These forms are the students' responsibility to complete in a timely manner!
At the end of your time off-campus, fill out the required “Program Evaluation” form, which will be sent to you. This form is essential feedback for our office and future OCS students about the academic and non-academic elements of your experience. It is required, and failure to complete this form will result in a delay in credit transfer.
