Medical Records & Confidentiality
Student medical records will be kept for 6 years following graduation from Bowdoin College.
Obtaining Medical Records:
To obtain a copy of your medical records, you will need to provide us with a completed medical records release form. This form may be obtained through accessing the link below, via email, fax, or mail. You may also obtain a records release form from your local health care provider.

As a professional courtesy, there is no fee for forwarding your medical records directly to another health care provider. There is, however, a minimum fee of $15 if a personal copy of records are requested.
Obtaining Immunization Records:
To obtain a copy of your immunization records only you will need to provide us with a completed immunization records request form, available at the link below, or by submitting a written request. There is no fee for copies of immunization records.

All medical records are confidential. Information contained in student medical records will not be released to anyone without permission. Information wil not be shared with parents, friends, professors, or college administrators without the patient's express permission. Students may be asked to give consent in writing.
