Programming Guide

Journal Vouchers can be found in the Student Activities Office (located behind the Smith Union Information Desk). Please do not hesitate to ask anyone in that office for help filling out the form or gathering the information listed below.

In order to fill out a Journal Voucher, you will need the following items:

  • The 6-digit project nuber of the donating organization or department
  • The 6-digit project number of the organization you are working for
  • The 4-digit account code describing what the money is to be used for
  • Supporting documentation (an email from the donating account's administrator giving the details of the donation is sufficient)
  • An authorizing signature from the budget managers of the associated account
