Enrollment and Tuition

The following forms must be completed for your child's initial enrollment in our program once you have received and signed an enrollment contract.

  • Emergency Contact Information Card
  • Child Health Assessment Form
  • Permission to Participate Form
  • Personal Information Form

Please make sure that all emergency information, your child's immunization forms, and health insurance information are kept updated at all times. If there are any changes, please let the caregivers in your program know.

The Re-Enrollment Process

If your child is currently enrolled, the following forms must be filled out at the beginning of each year:

  • Emergency Contact Information Card
  • Updated Child Health Assessment Form
  • Permission to Participate Form
  • Personal Information Form (only if your child is changing programs)
  • Enrollment Contract

For usability, portability, and for ease of printing, these forms are offered in PDF format. To view them, you will need software capable of reading PDF files. PDF software such as Adobe Acrobat Reader now comes standard on most Bowdoin computers and many personal computers around the world. If you do not already have such software, you may download the Acrobat Reader software for free from Adobe's website.
