If you are unable to work or attend school as a result of an illness or injury for a period of at least 60 days you may qualify for this deferment. You are also entitled to this deferment if you are unable to work or attend school for a period of at least 90 because you are providing care to a spouse or child for their temporary total disability.
To receive the deferment, you must submit a completed deferment request form, as well as an affidavit from your attending physician specifying the nature of your illness or injury, and the length of time your physician expects that you will be affected. The statement must also list his/her physician license number and the state in which it was granted.
